NCLEX Practice Question of the Week

Here is the weekly question posted on the NCSBN Learning Extension:

A charge nurse of a long term care (LTC) facility is making out assignments. Which assignment made to an unlicensed assistive personnel (UAP) requires intervention by the nursing director of the LTC facility?

  1. “Provide decubitis ulcer care and apply dry dressing to the site.”
  2. “Bathe and feed client who is on bed and chair rest.”
  3. “Oral suctioning as needed of an unresponsive older adult client.”
  4. “Review with family members the procedure of intermittent (bolus) feedings via G-tube.”

The correct answer is: 4 – review with family members the procedure of intermittent (bolus) feedings via g-tube

Apparently UAPs can do 1 and 3 on their own… where do they let that happen? 2 is something they already do. 4 is teaching, which from what has been drilled into my head, a nurse’s job.
