NCLEX Practice Question of the Week
Here is the weekly question posted on the NCSBN Learning Extension:
The nurse manager overhears a health care provider loudly criticize one of the staff nurses within the hearing range of others. The nurse manager’s next action should be to take what approach?
- Walk up to the health care provider and quietly state: “Stop this unacceptable behavior!”
- Allow the staff nurse to handle this situation without any interference.
- Notify the other administrative branch of a breach of professional conduct.
- Request an immediate private meeting with the health care provider and the staff nurse.
First Delivery
“Okay, who would like to see a delivery? Patient came into the ER last night and now her contractions are about four minutes apart. So if it doesn’t happen now, you’ll definitely see something before we leave. I talked to the mother and she agreed to let one of you watch, but it has to be a female. Sorry S.”
There were three of us in the room at the time, and one was already knocked out. I looked over at my classmate and urged her to go. She wants to go into maternity, and I’ve seen a couple of c-sections already.
“No, ST… you go. I’ll just wait for the next one.”
My clinical instructor sent me off to the lounge where I had to change into another set of scrubs. Unlike the mandatory school uniform, the L&D scrubs fit better and were much more comfy. I headed back to the confrence room and waited for the rest of the group to show up.
“Hey, how come you’re in different scrubs?”
“ST’s going to watch them deliver a baby!”
I wasn’t going to do anything, just watch. So why was I getting nervous?
Our clinical instructor came back in and gave us the rundown for the day. I would be in L&D with the mother until she was moved to the other side of the floor. They didn’t have many patients over in Maternity, so people would have to double. Postpartum assessments, and give whatever medications are indicated.
“Ready to go, ST?”
NCLEX Practice Question of the Week
Here is the weekly question posted on the NCSBN Learning Extension:
A charge nurse of a long term care (LTC) facility is making out assignments. Which assignment made to an unlicensed assistive personnel (UAP) requires intervention by the nursing director of the LTC facility?
- “Provide decubitis ulcer care and apply dry dressing to the site.”
- “Bathe and feed client who is on bed and chair rest.”
- “Oral suctioning as needed of an unresponsive older adult client.”
- “Review with family members the procedure of intermittent (bolus) feedings via G-tube.”
NCLEX Practice Question of the Week
Here is the weekly question posted on the NCSBN Learning Extension:
A client on telemetry begins to have premature ventricular beats (PVBs) at 12 per minute. A nurse reviews the most recent laboratory results. Which lab test would require immediate action by the nurse?
- “Calcium 9 mg/dL”
- “Magnesium 2.5 mg/dL”
- “Potassium 2.5 mEq/L”
- “Partial thromboplastin time 70 seconds”
Hooray!
99% on the re-take!
The only one I got wrong? Something really easy, actually.
t.i.d = three times a day
What did I write down? Twice a day. Which is b.i.d.
Silly, ST.
Sitting in the library right now with some classmates. Copying the required readings for our Nursing 2 final… which is Friday. Plenty of time to prepare, and get ready for the next class.
Yaaaay…. passed! Time to run.
“Just Breathe…”
Three classes down already. So crazy.
Before I can even think about my next class (Maternity-Pediatrics) I need to pass the medical calculations test.
I already failed one. This is my last chance. I don’t know what to do now. I’ve done practice questions. And some more. And some more. For the most part I get it, but then I get the string of stupid mistakes and of course the insecurities set in and my mind starts reeling and thinking about a plan B. I’ve been on the brink of tears a handful of times, and indulged in vices I keep swearing to give up.
Please… whoever is out there… help me get through this.
A Slight Spring In My Step
PHARMACOLOGY IS FREAKING OVER!!
Okay, now that I got that out of my system… let’s continue.
NCLEX Practice Question of the Week
Here is the weekly question posted on the NCSBN Learning Extension:
A client is to receive three doses of potassium chloride 10 mEq in 100 mL in 0.9% normal saline to infuse over 30 minutes each. Which action is a priority assessment to perform before a nurse gives this medication?
- “Oral fluid intake”
- “Bowel sounds”
- “Grip strength”
- “Urine output”
Quick Hello
Currently studying for the exam onslaught.
Med calculations to start the day off. Followed by lab demos and then pharmacology final. And right after that I will be whisked away to the skills lab for a certification.
But it doesn’t end there! Upon passing/failing the certification, I have to meet up with my group for our presentation that is due next week and studying for the second exam for Nursing 2.
When all is said and done, I will end the night with care plans and an early bedtime, since I have to be in the hospital for change of shift.
Hell week indeed.
NCLEX Practice Question of the Week
Here is the weekly question posted on the NCSBN Learning Extension:
A client calls the evening health clinic to state “I know I have severely low sugar since the Lantus insulin was given three hours ago and it peaks in two hours.” What should be the nurse’s initial response to the client?
- “What else you do you know about this type of insulin?”
- “What are you feeling at this moment?”
- “Have you eaten anything today?”
- “Are you taking any other insulin or medication?”
